3. Service requests are not confirmed appointments
Sending this form is a service request, not a confirmed appointment. Our dispatcher will contact you to confirm a time. Requests received after 5:00 PM or on weekends are returned the next business day. Submitting a form does not create a contract for service. Prices, scope and scheduling are confirmed with you directly before any work begins.

Membership plans described on this site are summaries. A membership enrollment request on this website is a request only. The full terms of any membership — including renewal, billing, commitment, cancellation and credits — are governed by the membership agreement you sign with us. If anything on this website differs from your signed agreement, the signed agreement controls.

Financing is provided by third-party lenders and is subject to their application and approval. Terms are set by the lender — ask our office for current details.
